Add ability for a user to leave an account

Chris Oliver   I think this should be part of Jumpstart. It is pretty much standard on all apps.

Requirements roughly would be:

- delete account_user button that appears to current_user only 
- email confirmations sent to both user and account owner 
- handle the case when user has no accounts, for example if user is a member of a single account and leaves it.
